The system is set up to have drug manufacturers compete to give the highest rebate to the pharmacy benefit managers (PBMs) (the middlemen who decide which drugs are covered by Medicare) in exchange for a preferred placement on the list of drugs covered by Medicare, known as the formulary, which determines plan coverage and the out-of-pocket cost for patients. This has led to increases in insulin list prices, rather than the decreases we should be seeing with such market competition, because PBMs are keeping rebates for themselves rather than passing them on to consumers. This increase is in addition to patient cost-sharing being based off the list price of insulin rather than the net price, which The Insulin Cost Reduction Act changes.\n\nGrothman\u2019s bill directly addresses the power PBMs hold over the market price of insulin. Instead of PBMs receiving rebates in the insulin space, The Insulin Cost Reduction Act would ban the use of rebates, unless the savings are passed on directly to patients, which in turn would lower costs at the pharmacy.\n\n\u201cThe Insulin Cost Reduction Act will help lead to patients with diabetes paying less at the counter for their treatment. By establishing the drug price after rebates are taken into account, patients will also see decreases in deductibles and copays,\u201d said Grothman. \u201cFor far too long, Pharmacy Benefit Managers have been incentivized to work against the best interest of consumers. Americans with diabetes rely on insulin for their well-being, and thanks to the system currently in place, consumers are seeing higher prices. This toxin has been a thorn in the side of the Drug Enforcement Agency and Customs and Border Protection (CBP) for decades. This year, fentanyl has been flooding into the United States in record numbers after President Biden reversed numerous immigration policies of the Trump Administration that were showing positive results. Border Patrol agents have told me that the cartels intentionally overflow certain areas of the Southern border, diverting resources, and creating vulnerable spots in other areas along the border that would otherwise be guarded. If the Biden Administration adequately supported CBP, these agents would be more prepared to detect and confiscate the copious amount of fentanyl entering our country. The lack of leadership from the White House concerning the border is encouraging drug traffickers to profit from the Administration\u2019s open border policies. It is an ongoing tragedy that Americans are paying the price of bad immigration policy in tens of thousands of human lives. Fentanyl is tearing families apart in what has become the deadliest drug crisis in American history. Making matters worse, NBC News reported that preliminary biometric screening capabilities were not in place at Hamid Karzai International Airport and CNN quoted a Customs and Border Protection official who characterized the White House priority as \"get[ting] as many people on the plane as you can, and we'll sort out the (immigration visa) stuff later.\" The same source added that \"some people have landed with no documents whatsoever, creating a very challenging work environment for the officers.\"\n\nMy office has assisted, since this crisis began, with getting those Afghans who participated in sensitive U.S. military, diplomatic and intelligence activities out of the country. I strongly support admitting these individuals to the United States and will continue to assist with that effort however I can. The Restoration of Employment Choice for Adults with Disabilities Act ensures that CRPs may continue to employ and pay their employees under a 14(c) certificate even if their state does not provide the required counseling.\n\nFAQ about CRPs\n\nQ: Don't individuals with disabilities deserve enough money to live?\n\nA: Yes. However, individuals who work at CRPs under 14(c) certificates have access to benefits like Supplemental Security Income (SSI) and Medicaid.\n\nQ: Why do these individuals work at a CRP if they don't need the money?\n\nA: For anyone, having a steady job and earning a paycheck is a point of pride. These individuals deserve the dignity that comes from feeling productive and contributing to society. It is also important for these individuals to establish strong friendships outside of their family. There was a time when these individuals did not socialize much outside of their family. At CRPs, they establish friendships both with individuals similar to themselves as well as the management staff that can last for years.\n\nQ: Why don't these individuals get a job outside of the CRP?\n\nA: Many do! CRPs are set up to assist individuals with disabilities in their work and teach them valuable skills, including how to operate in the workplace. Many people work for a CRP for a period of time, then use those skills to get a job elsewhere. Most people who do gain employment elsewhere, however, work much less than the 40 hours per week they may want. It\u2019s also true that people who do find jobs in the community can have their jobs terminated, and then it is important for them to have a fallback option, so many retain their job at the CRP in addition to working part-time for another employer.\n\nQ: Shouldn't individuals with disabilities be paid $15/hour instead of the CRP's profits going toward making the executives richer?\n\nA: CRPs are most often non-profit and money they do make is reinvested back into the company to assist the employees. I have been told separately and on more than one occasion by different CRPs in Wisconsin's Sixth District that if government regulations require CRPs to pay $15/hour, they would likely be forced to lay-off all employees and close their doors permanently.\n\nQ: Why would it be bad if CRPs closed permanently?\n\nA: The individuals who work there would lose their job and many would be relegated to adult daycare. But, if you want to get to the truth of what Tony Evers did that week, all you need to do is look at his own words.\n\nOn August 23, just hours after the shooting of Jacob Blake, but before rioting occurred, Tony Evers issued the following statement: \"Tonight, Jacob Blake was shot in the back multiple times, in broad daylight, in Kenosha, Wisconsin.\" He continued \"(w)hile we do not have all of the details yet, what we know for certain is that he is not the first Black man or person to have been shot or injured or mercilessly killed at the hands of individuals in law enforcement in our state or our country.\" Those were Governor Evers' incendiary written words in the immediate aftermath of the shooting. Without the facts, he immediately put the shooting in the context of race and insinuated this was a merciless act.\n\nEvers' statement and his public comments in the days that followed were so offensive and so dangerous that the heads of the Wisconsin Sheriffs' Associations and Police Chiefs' Association sent a letter to Evers. The letter urges the Governor to \"refrain from making statements specific to Kenosha\" because they put the lives of law enforcement officers and the public at risk. Stop and think about that. The organizations representing the Sheriffs and the Police Chiefs of this state banded together to tell our Governor to stop talking about Kenosha because his words put lives in danger.\n\nEvers also wants us to forget his failure to ask for federal help. Today, Evers defenders repeat the mantra that the Evers Administration filled \"all requests for assistance we received via official channels.\" That bit of bureaucratic word parsing may fool some, but we know what it means. They put in the bare minimum effort. What that meant was 125 Wisconsin National Guard troops on August 24, and 125 more on August 25. Those numbers were far short of what was needed. With Evers' lack of urgency, National Guard troops from other states did not arrive for days.\n\nWe are talking about life and death events playing out on our streets, yet the response sounds like something you would get from the DMV for failing to check a box on a form. In a crisis we expect more from our leaders. Forget \"requests received via official channels.\" The Governor needed to turn on a TV, check his iPhone, or, heaven forbid, get out of Madison and come to Kenosha to see what was really needed on the ground. It took Tony Evers four days to even come to the city.\n\nWe will never know how things may have played out if Evers acted differently on day one. If Evers had responsibly toned down the rhetoric and demanded calm. If Evers called for a detailed and thorough investigation of the shooting. If Evers made clear that the streets of Kenosha would immediately have all the aid necessary to maintain order. What we do know is that Evers chose the opposite path, throwing rhetorical gasoline on the fire.\n\nAnd, we know what happened next.\n\nTwo days later, August 25, Evers' words were even worse. Earlier that day, city and county leaders, as well as local law enforcement officials, made an urgent call to me asking for assistance. As a federal official, it was obvious they were seeking federal help, but really, they were looking for any help they could get. Knowing what was happening in Kenosha, and hearing their urgency, I called the White House and asked to speak with President Trump. The President took my call. He explained that Governor Evers had not asked for help, but promised to offer federal assistance to Kenosha.\n\nShortly after that call, the President reached out to the Governor. Shockingly, Evers refused the help.
