The funds will help support efforts aimed at coordinating patient health care through patient-centered medical homes.\r\n\u201cOur West Virginia families and children all across our state, including our most rural communities, deserve to have access to the best quality healthcare services,\u201d Manchin said. \u201cThese grants will help make much needed upgrades and improvements to our health centers so that our West Virginia health care professionals can provide top-of-the-line service and care to all of our residents.\u201d\r\n\u201cIn our rural communities, patient-centered medical homes have become a great resource for our health centers in their work to improve access to care and health outcomes in underserved areas,\u201d Rockefeller said. \u201cThis investment in several health centers across West Virginia will provide additional resources for medical homes and their important work to coordinate patient care and educate patients on how to be an active partner in their care.
