While Congress must address the looming Medicaid funding cliff through legislation, we believe there are steps CMS can take to provide immediate relief without Congressional action. For example, CMS can use its authority to address how displaced Medicaid beneficiaries access care from out-of-network providers on the mainland. Additionally, following the declaration of a Public Health Emergency, CMS can delay Medicaid redeterminations, streamline program eligibility, and waive or modify other program requirements. In Puerto Rico, seniors have enrolled in MA plans at a rate nearly double that of any state. Despite having higher enrollment rates and often higher input costs, MA payment rates in Puerto Rico are currently 43 percent below the national average. This underfunding aggravates larger systemic problems, including lower provider reimbursement rates and high-levels of migration off the Island by providers leading to shortages. In its 2016 report, the Congressional Task Force on Economic Growth in Puerto Rico recommended that CMS consider whether additional administrative steps are warranted to ensure that MA plans, including those in Puerto Rico, are being fairly and properly compensated for the services they provide to beneficiaries. CMS can also recalculate Puerto Ricos Medicare Disproportionate Share Hospital payments to account for the fact that residents of Puerto Rico are ineligible for Supplemental Security Income. In light of the recent devastation after Hurricane Maria, we believe these additional steps are ripe for action. We urge CMS to follow through that bipartisan, bicameral recommendation and take administrative action in the upcoming 2019 Advanced Notice to address the significant disparity between MA payments rates in the 50 states versus Puerto Rico. Florida Agriculture: 2018 will be a year that will deal with the farm bill. And I hope action will be taken to reform crop insurance to ensure that my states farmers are never in the position they have been put in after Hurricane IrmaOn reforming social safety net: You have to have a safety net to take care of those who cannot take care of themselves: the permanently disabled, the elderly, and the like. But you also have to have a safety net for the people who have come upon tough times, until they can get back on their feet and try again. But I fear, in fact, I have realized long ago that our safety net programs treat the symptoms of poverty, but they do not cure it. It will not be so much about cutting as it will about reorganizing and improving. Yes we will take care of people in their emergent needs and immediate needs. Therefore, we urge your Administration to advance a new focus on Hezbollahs drug trafficking efforts and connections to organized crime in the region by directing U.S. agencies to designate Hezbollah as a Transnational Criminal Organization (TCO) under E.O. 13581 and as a Specially Designated Narcotics Trafficking Kingpin (SDNTK) entity pursuant to the Foreign Narcotics Kingpin Designation Act. These events follow the 2014 arrest of a Hezbollah operative in Peru, the Iranian-linked assassination attempt on the Saudi Ambassador in 2011, and the terrorist attacks against two Jewish targets in Argentina in the 1990s as well as the assassination of former Special Prosecutor Alberto Nisman in 2015, who uncovered evidence of Iran and Hezbollahs role in these attacks. Darren Soto to rename a Kissimmee post office for the Puerto Rican Borinqueneers U.S. Army regiment now is getting pushed in the U.S. Senate by Floridas U.S. Sens. Marco Rubio and Bill Nelson. The bill, House Resolution 4042, intends to rename the Kissimmee post office to honor the U.S. Armys 65th Infantry Regiment, first established in Puerto Rico in 1898, and which was recognized for fighting with valor in World War II and the Korean War among others. The regiment was known as the Borinqueneers, named after the historic inhabitants of the island. In 2014 the Borinqueneers were honored with Congressional Gold Medals. Soto, an Orlando Democrat with Puerto Rican heritage whose district includes Kissimmee, introduced the bill in October and it was approved by unanimous consent in the U.S. House of Representatives last Thursday. Independent media organizations have been shuttered, and dozens of journalists, bloggers, and citizens have been jailed for their speech. The Egyptian government has been engaged in a wide-ranging effort to crack down against public discourse, civil society activity, and independent media using a variety of methods and tools, including the use of legislation ostensibly designed to promote public safety or regulation, but practically used to stifle dissent. In particular, the NGO law ratified by President al-Sisi in May 2017 threatens to devastate the ability of independent organizations to operate within Egypt and calls into question the Egyptian Governments own commitments to freedom of expression and freedom of assembly. The government routinely pressures journalists and private citizens to suppress criticism of the government, and has used criminal prosecution, gag orders, arbitrary detention, and serious physical abuse in police custody to target free speech. The criminalization of legitimate NGO activity complicates the effectiveness of the foreign assistance package that the United States provides to Egypt annually. The usage of these funds, and their compliance with U.S. law under the Brownback Amendment, relies on the ability of implementing partners on the ground to function freely. Recently, the Egyptian government has participated in the disturbing global trend of government authorities targeting directly, as well as fostering an environment of discrimination and violence against, members of the LGBT community. The most recent wave of anti-LGBT persecution has been ongoing since September, when authorities in Cairo arrested seven people who had been photographed at a concert raising a rainbow flag. These arrests marked the beginning of a widespread crackdown on LGBT Egyptians with reports of 65 people being arrested. Most have been charged with sentences ranging from 6 months to 12 years in prison. Reports that the Egyptian Parliament will consider legislation that would criminalize same sex relations are further cause for concern. While the Egyptian government has demonstrated an interest in achieving greater parity for Coptic Christians, Copts continue to be the frequent target of systemic discrimination as well as violence carried out by non-state actors, including fellow Egyptians. Despite statements of support, the government has not taken sufficient steps to protect Copts from violence and ensure equal treatment under the law. Finally, the Egyptian government has repeatedly stated its intent to find a satisfactory resolution in the case against 43 NGO workers, including 19 Americans, who were unjustly convicted under the Morsi government in 2013. This issue has been an irritant in U.S.-Egyptian relations for too long and has set a troubling precedent with regard to Egypts treatment of democracy, human rights, and governance efforts, including programs funded by U.S. tax dollars. We urge that this language be included in the final agriculture appropriations bill. As you know, this provision will prevent the reopening of horse slaughter facilities in the United States by prohibiting federal funding for health and safety inspections, which are required by law at all meat processing plants. The U.S. Congress has adopted this provision nearly every year since 2005, ensuring that the horse slaughter industry does not return to American soil. Due to this strict funding prohibition, no slaughterhouses have operated in the United States since 2007. In fact, Congress just renewed this in May when it finalized the FY 2017 government funding bill. There is no market for horsemeat in America - it is the product of an inhumane business which 80% of Americans oppose. Communities that have lived with this industry are its strongest opponents; the two states in which the last three horse slaughterhouses operated have enacted laws banning the industry from returning. Those foreign-owned operations regularly received citations for polluting surrounding groundwater and air, strained the budgets of their host cities, and created a nuisance for nearby residents. American taxpayers should not have to support an industry that they overwhelmingly oppose, and which primarily benefits foreign companies and foreign markets. Furthermore, meat from American horses presents significant risks to public health. Horses are routinely given a range of drugs and chemicals banned by the U.S. Food and Drug Administration (FDA) for use on animals meant for human consumption. In 2015, the European Union implemented a ban on horsemeat imports from Mexico after EU inspectors consistently found drug residues in the product. The U.S. Department of Agriculture (USDA) is now experiencing a severe shortage of veterinarians to inspect commercial slaughter facilities. Activating new facilities to slaughter horsemeat for export would further strain the inspection system, threatening the safety of the food that Americans do consume, and creating a serious risk of horsemeat comingling with other commercial meat products.   Congress is asked to make tough decisions about providing resources for important programs. Horse slaughter threatens public health, is strongly opposed by the vast majority of Americans, has no American market, and benefits foreign interests.
