Background: In October of 2012, due to staffing shortages and questionable administration of care, the Fort Wayne VA put a pause on offering inpatient services. In response, the next month Congressman Stutzman and then-Congressman Donnelly requested the VA Office of the Inspector General review of the overall quality of care and management at the Fort Wayne hospital, investigate the issues leading to the suspension of inpatient care, and determine what measures would be needed to reopen the facility to normal operations.
