It is unacceptable that the VA has neither followed nor enforced its own policies to protect veteran patients from doctors who are unfit to provide care. The VA must follow through on its pledge to investigate the circumstances that led to these failures and prevent them from happening again. Clearly, we still have a long way to go to truly reform the VA.
